垂体后叶素在脓毒性休克治疗中的疗效观察

目的:观察垂体后叶素在脓毒性休克治疗中对血流动力学和预后的影响。方法:将69例脓毒性休克患者随机分为去甲肾上腺素组(A组)和垂体后叶素组(B组)。均按EGDT方案(早期目标指导性治疗方案)进行复苏治疗,分别应用去甲肾上腺素及垂体后叶素,动态观察6、24、72h心率、平均动脉压(MAP)、中心静脉压(CVP)、尿量、血pH值及氧合指数,并对心律失常检出率、ST段移位发生率、28及90d病死率进行对比观察。结果:治疗6h后MAP、治疗24h后MAP,CVP,72h后氧合指数、CVP等指标,B组优于A组(P<0.05)。心律失常检出率、ST段移位发生率、28、90d病死率2组无明显差异。结论:垂体后叶素在治疗脓毒性休克、稳定血流动力学方面可能优于去甲肾上腺素。

Observation on the therapeutic effect of pituitrin in treatment of septic shock

Objective:To observe the effect of pituitrin on hemodynamics and prognosis in treatment of septic shock.Methods:Sixty-nine patients were randomly divided into two groups:noradrenaline group(group A) and hypophysin group(group B).Patients were treated with schema EGDT(early goal-directed therapy),and were given noradrendline and hopophysin respectively.Index of heart rate、mean arterial pressure(MAP),central venous pressure(CVP),urine volume,blood pH,oxygenation index were observed dynamically in 6,24,72 hours respectively,and the detection rate of arrhythmia,shifting of ST,mortality at 28 and 90 days were compared.Results:The index of MAP and blood pH treated after 6 hours,MAP and CVP treated after 24 hours,oxygenation index and CVP treated after 72 hours in group B is superior to those in group A(P<0.05).There was no significant difference in the detection rate of arrhythmia,shifting of ST,mortality at 28 and 90 days between two groups.Conclusions:Pituitrin is superior to noradrenaline in treating septic shock and stabling the hemodynamics.
