胰岛素泵治疗2型糖尿病合并感染临床分析

摘    要:目的:探讨胰岛素泵治疗2型糖尿病合并感染患者的临床疗效。方法:48例2型糖尿病合并感染患者随机分为观察组和对照组两组,每组24例,在抗感染治疗同时,观察组使用胰岛素泵持续输入诺和灵(R)控制血糖,对照组予皮下注射胰岛素(诺和灵R,诺和灵N)强化控制血糖,并记录两组治疗病程中低血糖总次数,比较两组治疗前及入院第2天、第2周、第3周空腹、餐后2h血糖水平及每天胰岛素平均用量,并比较血糖达标时间、低血糖的发生情况。结果:两组治疗后血糖控制良好,治疗前及治疗后第2、3周空腹血糖、餐后2h血糖差异无统计学意义(P>0.05),第2天观察组血糖水平明显低于对照组(P<0.01);观察组各时间点血糖达标时间较对照组明显缩短(P均<0.01),达标时各胰岛素用量均明显少于对照组(P均<0.01),低血糖发生人次明显少于对照组(P<0.01)。结论:胰岛素泵能减少胰岛素用量,缩短血糖达标时间,减少低血糖等不良反应,疗效相当于多次皮下注射胰岛素,减轻患者痛苦,值得临床推广。

Abstract:Objective: To explore the effect of insulin pump on type 2 diabetes complicated with infection. Methods: A total of 48 cases with type 2 diabetes and concurrent infection were randomly divided into observation group and control group by half. Besides, anti infection treatment, patients in observation group also received continuous injection of Nuoheling by insulin pump to control blood glucose, while the control group had subcutaneous injection of insulin. Indexes including the level of fasting blood glucose and postprandial glucose 2 h after dinner, average dosage of insulin per day, recovery time and the incidence of hypoglycemia before admission, on the 2nd day, the 2nd and 3rd week after admission were com pared. Results. The level of blood glucose in both groups were under satisfactory control. There was no significant difference in fasting blood glucose and postprandial blood glucose either before treatment, or on 2nd and 3rd week after admission between two groups (P〉0.05). While on the 2nd day after treat ment, the level was significant lower in observation group (P〈0.01). Cases in observation group had significant shorter recovery time (P〈0.01), less insulin (P〈0.01) and lower incidence of hypoglycemia at all observation times (P〈0.01). Insulin pump is as effective as multiple subcutaneous injection of insulin. It can reduce the dosage of insulin, shorten the recovery time, reduce side effect, relieve the pain, and deserves clinical application.
