唇裂修复术对唇腭裂患者上颌骨及面部软组织发育的影响

摘    要:目的分析单侧完全性唇腭裂患者唇裂手术对其上颌骨生长发育及面部软组织发育的影响。方法选择单侧完全性唇腭裂患者(UCLP)均已行手术者109例为研究对象。采用头影测量和定位拍照分析上颌骨生长发育及面部软组织发育,并与正常对照组同龄者分组进行比较。结果UCLP的唇腭裂均修复组与仅唇裂修复组,上颌骨相对于颅底均有明显的后退,并且上颌骨长度较正常对照组显著变小。软组织测量中,UCLP的上唇面积、上自唇高、上唇长度、口裂宽度较正常对照组减少;上红唇高差异无显著性。鼻底凸度、上唇凸度较正常对照组减少;下唇凸度差异无显著性。结论唇裂手术对UCLP上颌骨向前的生长有明显的抑制作用,对软组织发育的影响不明显。

Abstract:Objective To analyse the isolated effects of lip repair on inhibition of maxillary growth and facial soft tissue development in patients with complete unilateral cleft lip and palate. Methods The lateral cephalometric analysis and the front and lateral photographs were applied to analyse the maxillary growth and characteristics of upper lip in 109 patients with complete unilateral cleft lip and palate who had undergone surgical repair. Results Maxillary retrusion was obvious and maxillary length became smaller in all patients than that of normal subjects. The area and height of the upper lip and width of the mouth were smaller than normal size. The convexity of nose bottom and upper lip was reduced. Conclusion Lip repair can substantially inhibit maxillary growth in patients with complete unilateral cleft lip and palate, but the effect is not evident enough to affect upgrowth of the soft tissue.
