初诊系统性红斑狼疮患者血液系统异常的临床分析

目的:提高对伴有血像异常的初诊SLE的诊疗水平.方法:回顾性分析84例初诊SLE的临床资料.结果:在初诊的病例中,77例存在血像异常(91.6%),一系血细胞异常共27例,二系血细胞异常共32例,三系血细胞异常18例,白细胞减少、贫血多为轻、中度(68.9%,80.5%);贫血主要表现为小细胞性18例(23.4%)和正细胞性44例(57.1%);血小板减少程度的变异性大(11-97.8)×109/L.绝大部分病人均伴有SLE的其他症状.57例DAI(病情活动指数)数值大于9.结论:初诊SLE伴血液系统异常发生率高,SLE的血液系统表现有其特点,存在血液系统损害的初诊SLE病人中有74%伴有SLE病情活动.

Clinical Analysis of Abnormalities of Blood System in Newly Diagnosed Systemic Lupus Erythematosus

Objective： To improve the diagnosis and treatment of SLE patients who are newly diagnosed and with abnormal blood system. Method： The clinical data of 84 patients with newly diagnosed SLE were analyzed retrospectively. Results： 77 cases have blood system abnormal （91.6%）, 27 eases of single lineage abnormal, 32 cases are two lineages abnormal and 18 cases are three lineages abnormal. Leukopenia, anemia mostly mild to moderate （68.9%,80.5%）. Aceording to morphological classification of erythrocyte, there are 18 cases of Small Cell Anemia （23.4%） ; 44 cases of Normal Cell Anemia （57.1%）. The range of Thrombocytopenia is large degree of variability （11-97.8） ×10^9/L. The vast majority of patients with other symptoms of SLE. 57 cases of DAI〉9. Conclusion： There is high incidence of blood system abnormal in newly diagnosed SLE. SLE manifestations of the blood system has its own characteristics. Our analysis shows that the existence of the blood system damage in newly diagnosed SLE patients with 74% of SLE disease activity.
