以咽喉症状为主诉的桥本甲状腺炎45例报告

目的：探讨对以咽喉症状为主诉的桥本甲状腺炎的诊疗手段。方法：介绍45例临床资料，咽异物感17例，咽刺痛感5例，咽压迫感10例，咽干燥感7例，咽痛6例，其中4例还伴声嘶，37例甲状腺聚99mTc功能异常，8例正常，33例血清T3，T4水平异常，12例正常；35例检测血清TSH水平，26例异常，9例正常；TMAb和TGAb阳性率分别为66%（21/32）和56?8/32）；甲状腺针吸活检证实30例桥本甲状腺炎，7例桥本甲亢，所有患者均口服甲状腺片或丙硫氧嘧啶。结果：在随访32例中，27例半年内咽喉症状消失。5例好转。结论：咽异感症，声嘶，咽痛是桥本甲状腺炎不可忽视的症状，药物疗法可改善或消除咽喉症状。

Diagnosis and treatment in 45 patients with Hashimoto''''s thyroiditis associated with throat symptoms

Objective:To assess the diagnosis and treatment in the patients complaining of throat discomfort that resulted from Hashimoto's thyroiditis (HT).Method:Forty five patients were reported for their clinical dates, 17 had abnormal sensation of pharynx, 5 stabbing feeling, 10 compressive feeling, 7 dry feeling, 6 sore throat. The pharyngeal symptoms were accompanied with hoarseness in 4. Thyroid uptake of 99m Tc scan showed abnormal in 37 and normal in 8. Serum T 3?T 4 level was abnormal in 33 and normal in 12. Serum TSH level was abnormal in 26. Antithyroid antibodies estimated in 32 patients detected positive titers of TMAb and TGAb in 21 (66%) and in 18 (56%), respectively. Thirty were diagnosed as having HT by thyroid fine needle aspiration biopsy (FNAB), 7 Hashimoto's hyperthyroidism. Tab.Thyroidi were administrated to the patients at the period of euthyroidism or hypothyroidism and Propylthiouracilum to the ones at the period of hyperthyroidism.Result:Thirty two patients received the follow up for six months, throat symptoms disappeared in 27 and improved in 5.Conclusion:abnormal sensation of pharynx and hoarseness as well as sore throat play an important role in symptomatology of HT. Medicine therapy may make the patients with HT free of throat symptoms.
