脂蛋白(a)和C反应蛋白水平与维持性血液透析患者左室功能的关系

摘    要:目的探讨脂蛋白(a)[LP(a)]和C反应蛋白(CRP)水平与维持性血液透析(MHD)患者左室功能的关系。方法对71例MHD患者测定其血清CRP、LP(a)水平,根据LP(a)、CRP水平将患者分为LP(a)升高组[LP(a)≥300 mg/L]、CRP升高组[CRP≥8 mg/L],LP(a)正常组[LP(a)<300 mg/L]、CRP正常组(CRP<8 mg/L),所有患者用心脏彩色超声波测定左室功能的各项参数,并分别进行左室功能的评价。结果与左心功能正常组比较,左室功能不全组血清LP(a)和CRP水平明显升高(P<0.01)。结论 MHD患者存在慢性炎症状态,慢性炎症状态参与了MHD患者左室功能不全的发生。

Abstract:Objective To study the relationship of left ventricular function and serum lipoprotein(a) and c-reactive protein(CRP) levels in maintenance hemodialysis(MHD) patients.Methods The serum levels of CRP and LP(a) in 71 patients with maintenance hemodialysis were determined.According to the CRP and LP(a) levels,the patients were divided into elevated LP(a) group(LP(a)≥300mg/L) and elevated CRP group(CRP≥8mg/L) and normal LP(a) group(LP(a)〈300mg/L) and normal CRP group(CRP〈8mg/L).The function and structure of left ventricular in all patients were analyzed by the color Doppler ultrasonography.Results Compared to the MHD patients with normal lefe ventricular function,the serum LP(a) and CRP levels in MHD patients with left ventricular dysfunction increased significantly(P〈0.01).Conclusion Microinflammatory state existes in patients with maintenance hemodialysis participates in the development and the progression of cardiorascular complications in MHD patients.
