手术室接错病人或做错手术部位的风险分析及对策

目的]探讨手术室接错病人或做错手术部位的风险因素,并提出有效的核查措施。方法]手术室巡回护士、主管医生、麻醉医生共同核对病人、病历及电脑打印手术通知单,应用"手术病人接送卡"和"术前核对表",并在接送床床头悬挂手术间号牌,防止接错病人或做错手术部位。结果]6年来未发生此项差错事故,提高了手术护理工作质量。结论]加强护理安全及法律意识,严格执行核查制度和交接班制度,是保障护理质量的根本。

Analysis of the risks of picking up patients wrong or doing wrong operative site in operating room and its countermeasures

Objective：To probe into the risk factors of picking up patients wrong or doing wrong operative site in operating room and to put forward some effective verification measures.Methods：Circulating nurse,responsible doctor and anesthesia doctor in operating room checked patient,medical record and computer printed operating notification sheet together.They applied ＂shuttle card for operating patient＂ and ＂preoperative checking card＂.And they hanged the number card of operating room on the ahead of the bed so as to prevent from picking up patients wrong or doing wrong operative site.Results：No mistakes had occurred in the recent 6 years.The quality of nursing in operating room management had been enhanced.Conclusion：To strengthen nursing safety and legal awareness and to carry out check and shifting systems strictly are the fundamentals to guarantee the quality of nursing.
