泌尿系小细胞癌3例的诊断与治疗

目的探讨泌尿系小细胞癌的诊治疗效。方法对3例泌尿系小细胞癌的病理及临床资料进行回顾性分析。结果3例患者,男性1例,女性2例,平均年龄71岁,位于膀胱2例,输尿管1例。分期T2N0M02例,T2N1M01例。2例行根治性膀胱切除术+化疗,1例行肾、输尿管及膀胱袖套样切除术+放疗+膀胱灌注。3例均获随访,1例术后9个月死于肺转移,2例无瘤生存至今,继续随访中。结论泌尿系小细胞癌恶性度高,预后差,手术联合放化疗可能提高其治愈率。

Diagnosis and treatment of small cell carcinoma of the urinary tract (report of 3 cases and literature review)

Objective To study the diagnosis,treatment and prognosis of small cell carcinoma of urinary tract. Methods A retrospective study of pathological and clinical data of 3 urinary small cell carcinoma cases was made. Results The three urinary small cell carcinomas(one in ureter and two in bladder ) were diagnosed in 1 male patient and 2 female patients with a mean age of 71 (70～74).The stages were T2N0M0 (2cases),T2N1M0 (1 case).2 patients were treated with radical cystectomy + chemotherapy,1 patient was treated with nephroureterectomy,together with the removal of a"cuff"of the bladder + radiotherapy + bladder irrigation. 3 patients were followed up,and 1 case die of lung metastasis 9 months postoperation,the other 3two cases were survived by now. Conclusion The urinary small cell carcinoma has a very poor prognosis. Multidrug combination chemotherapy and radiotherapy combined with surgery appears to be the treatment of choice.
