后路钛缆固定治疗小儿齿突骨折伴寰枢椎脱位的疗效观察

目的评估后路钛缆固定治疗小儿齿突骨折伴寰枢椎脱位的手术疗效，讨论小儿齿突骨折的诊断及治疗。方法对8例齿突骨折伴寰枢椎脱位的患儿行后路髂骨植骨钛缆固定术，术后头颈胸石膏外固定，回顾性分析其疗效。结果平均随访24个月，8例患儿植骨全部融合，3例遗留颈部僵硬感，无鹅颈等发育性畸形出现。结论颈后路钛缆固定操作简便安全，植骨融合率高，并发症少，可为小儿齿突骨折伴寰枢椎脱位非手术治疗无效时的首选治疗方法。

Application of titanium cable in the treatment of odontoid fracture with atlantoaxial dislocation in children

Objective To evaluate clinical result of titanium cable fixation for odontoid fractures with atlantoaxial dislocation in children of which both diagnosis and treatment were discussed. Methods Eight children of odontoid fracture with atlantoaxial dislocation were analyzed retrospectively after they were treated with posterior iliac bone graft, fixation of titanium cable and postoperative head-neck-thoracic plaster fixation from March 2007 to May 2008. Results With an average follow-up of 24 months, all 8 cases of patients have bony fusion. Neck stiffness occurred in 3 cases, and no developmental abnormalities such as gooseneck existed. Conclusion Titanium cable fixation is easy and safe with few complications. It provides good fixation leading to high rate of graft fusion. It can be the first choice of the treatment of odontoid fracture with atlantoaxial dislocation in children when conservation methods fail.
