不同抑郁程度对脊柱转移癌患者临床症状及生存的影响

目的:研究不同抑郁程度对脊柱转移癌患者临床症状及生存的影响.方法:采用Zung抑郁自评量表对我科1999～2001年125名脊柱转移癌患者进行问卷调查,并研究不同抑郁程度与临床因素及生存期之间的相关性.结果:44％(55/125)的患者存在不同程度的抑郁状态,其中轻度抑郁11.2％(14/125)、中度抑郁25.6％(32/125)、重度抑郁7.2％(9/125).平均随访时间21个月.无抑郁组生存期明显长于中、重度抑郁组(P<0.01).抑郁组疼痛程度、疼痛持续时间、疲劳及不良反应频繁例数均高于无抑郁组(P<0.01);同时中、重度抑郁组疼痛时间、不良反应频繁及瘫痪例数均明显多于轻度抑郁组(P<0.01).结论:在脊柱转移癌患者的治疗中应充分重视抑郁的早期诊断和治疗.

Influence of different depression status on symptoms and survival status in patients with metastasis tumor in spine

Objective:To investigate the influence of different depression degree status on the clinic symptoms and survival status in the patients with metastasis tumor in spine (MTS). Methods: By using Zung's self rating depression scale (SDS), we studied 125 patients with MTS between 1999 and 2001. Results: It was found that 44% patients with MTS were affected by depression. The survival period in the negative depression group(NDP) was longer than that in the moderate and the severe groups after a mean of 21 months follow up( P <0.01). The degree and the duration of the pain, fatigue, and the frequency of adverse reaction in depression group(DP) were more serious than those in the NDP group( P <0.01). Meanwhile, the duration of the pain, the frequency of adverse reaction and paralysis in the moderate and the severe DP were more serious than those in the mild DP( P <0.01). Conclusion: More attention need to be paid to both early diagnosis and treatment of depression in MTS patients.
