喹硫平治疗精神分裂症伴抑郁症状的对照研究

目的:研究喹硫平与利培酮在治疗首发精神分裂症伴抑郁症状的临床疗效和安全性。方法:74例符合CCMD-Ⅲ的首发精神分裂症患者,PANSS(阳性及阴性症状量表)评分≥60分,PANSS抑郁症状4个因子至少一个因子分评分≥4分,随机分为喹硫平组和利培酮组,治疗6周,采用PANSS评定疗效,采用PANSS因子分评定抑郁焦虑症状,采用治疗中出现的症状量表(TESS)评定不良反应。结果:喹硫平组总有效率为92.1%,利培酮组为94.0%,两组差异无统计学意义(P>0.05),喹硫平和利培酮的PANSS抑郁因子分在治疗4周后均有下降,两组差异有统计学意义(P<0.05)。喹硫平不良反应为嗜睡,体重增加。利培酮有锥体外系反应、泌乳、月经紊乱、体重增加、激越等。结论:①喹硫平和利培酮治疗首发精神分裂症均有良好疗效;②对于伴有抑郁症状的精神分裂症患者,喹硫平组优于利培酮组;③两组药物不良反应均轻。

A control study of quetiapine and resperidone in the treatment of schizophrenia with depressive symptoms

Objective To explore the clinical efficacy and safety between quetiapine and resperidone in the treatment of first- episode schizophrenia with depressive symptoms. Method Total 74 first - episode schizophrenia patients with depressive symptoms were randomly treated with quetiapine or resperidone for 6 weeks. The clinical efficacy was measured with PANSS and the safety was evaluated with TESS. Results The total effective rate of quetiapine group was 92.1%, and that of resperidone group was 94. 0%, there was no significant difference between the two groups（ P 〉 0. 05 ）. Oar study showed that the depression factor scores of PANSS were both reduced after treat- ment with quetiapine and resperidone for 4 weeks, but there was significant difference between the two groups （ P 〈 0. 05 ）. The main side effects of quetiapine were drowsiness and weight gain, and those of resperidone were EPS, unwanted break milk, the end of menstruation, weight gain and agitation,and so on. Conclusion ①Both quetiapine and resperidone are effective in the treatment of first -episode schizo- phrenia②Quetiapine is superior to resperidone in the treatment of first- episode schizophrenia with depressive symptoms ③There is no sighificant difference in side - effects of the two groups.
