McDonald宫颈环扎手术效果的相关因素研究

目的 探究影响McDonald宫颈环扎手术效果的相关因素.方法 选取2012年1月至2015年6月于本院接受McDonald宫颈环扎手术的114例妊娠产妇进行回顾性分析,其中77例手术成功的产妇作为观察组,37例手术失败的产妇作为对照组.对两组产妇妊娠指标、子宫指标、手术方式以及妊娠期病症进行比较分析,利用Logistic回归分析探究影响McDonald官颈环扎手术效果的相关因素.结果 两组患者年龄、孕次、是否双胎、晚期流产史、阴道炎、妊娠期糖尿病、多囊卵巢综合征、低钙等指标比较差异无统计学意义(P＞0.05);观察组和对照组术前宫口扩大、术前宫缩、宫颈长度、感染、低钾血症、羊膜囊突出以及缝合方式比较差异有统计学意义(P＜0.05),且经单因素Logistic回归分析均差异有统计学意义(P＜0.05),其中术前宫口扩大、术前官缩以及宫颈长度三项进入多因素Logistic回归分析,经分析差异有统计学意义(P＜0.05).结论 McDonald宫颈环扎手术效果受多种因素影响,其中宫颈长度为保护性因素,宫口扩大和术前宫缩为危险性因素

Study on the related factors of the effect of McDonald cervical cerclage surgery

Objective To explore related factors influencing the effect of McDonald cervical cerclage surgery.Methods 104 pregnant women underwent McDonald cervical cerclage surgery in our hospital from January 2012 to June 2015 were analyzed retrospectively.Among them,77 cases operated successfully were selected as observation group,37 patients who failed in the operation were selected as control group.Pregnancy index,uterus index,surgical procedures,and pregnancy disorders of two groups were compared,explored factors affecting the outcomes of McDonald cerclage surgery with Logistic regression analysis.Results There were no statistically significant differences in age,the times of pregnancy,twins pregnancy,the history of abortion,vaginitis,gestational diabetes,polycystic ovary syndrome,low calcium between observation group and control group (P＞0.05).There were statistically significant differences in preoperative cervix expanding,preoperative uterine contraction,cervical length,infection,hypokalemia,amniotic sac protruding,and suturing method between observation group and control group (P＜0.05),with statistically significant differences analyzed by single factor Logistic regression analysis (P＜0.05).Multivariate Logistic regression analysis showed that preoperative cervix expanding,preoperative uterine contraction,cervical length were related factors influencing the effect of McDonald cervical cerclage surgery (P＜0.05).Conclusion The effect of McDonald cerclage surgery is affected by many factors,in which cervical length is a kind of protective factor,preoperative cervix expanding,preoperative uterine contraction are risk factors.
