婴幼儿先天性鼻泪管阻塞的治疗时机与方法

目的 探究婴幼儿先天性鼻泪管阻塞的治疗时机与方法.方法 收集2014年2月至2016年2月于本院门诊和住院部治疗的先天性鼻泪管阻塞患儿562例,划分为不同年龄段组,第一组,1天龄～3月龄285例335眼;第二组,3月龄～6月龄194例236眼;第三组,6月龄～ 24月龄83例102眼.分别行泪囊区按摩+抗生素眼液滴眼、泪道加压冲洗法和鼻泪管探通术进行治疗,观察三组疗效情况.结果 疗效情况相比,第二组(88.98％)与第三组治愈率(94.12％)明显优于第一组(43.28％)(x 2=81.71,122.95;P＜0.01);第二、三组治愈率相比,差异无统计学意义(x2=2.19,P＞0.05).结论 对于先天性婴幼儿鼻泪管阻塞的临床治疗,应按照年龄阶段进行划分治疗,而泪道加压冲洗可认为是婴幼儿先天性鼻泪管阻塞治疗的首选方法.

The treatment timing and methods of infants with congenital nasolacrimal duct obstruction

Objective To explore the treatment timing and methods of infants with congenital nasolacrimal duct obstruction.Methods 562 infants with congenital nasolacrimal duct obstruction admitted in the outpatient and inpatient department of our hospital from February 2014 to February 2016 were selected,and divided into three groups:group 1 (1 day old to 3 months old,285 cases,335 eyes),group 2 (3 to 6 months old,194 cases,236 eyes),group 3 (6 to 24 months old,83 cases,102 eyes).Three groups were separately treated with lacrimal sac massage + antibiotic eye drops,lacrimal passage pressure flushing,nasolacrimal duct probing.Observed the curative effect of three groups.Results The cure rate of group 2,3 were obviously superior to that of group 1 (88.98％ vs.94.12％ vs.43.28％;x 2=81.71,122.95,P＜0.01),there was no statistically significant difference in the cure rate between group 2 and group 3 (x 2=2.19,P＞0.05).Conclusions The clinical treatment of infants with congenital nasolacrimal duct obstruction should be conducted according to the age of infants.Lacrimal passage pressure flushing can be the first choice of treatment methods of infants with congenital nasolacrimal duct obstruction.
