延续护理对老年缺血性脑卒中患者出院指导依从性的影响

目的 研究延续护理在提高老年缺血性脑卒中患者出院指导依从性中的应用效果.方法 选取出院老年缺血性脑卒中患者72例,自愿分为对照组37例,干预组35例;对照组给予常规出院指导,干预组制定出院患者延续护理服务登记表,建立出院指导规范化健康教育体系(对基于二级预防密切相关5个方面),于患者出院前3～7天进行评估,制定健康计划,制作患者访视记录手册,于出院后1周末、4周末、12周末进行规范出院访视,并记录.结果 采用独立样本检验,比较两组患者出院后12周末的单项依从性及依从性总分,结果显示:依从性总分干预组为(9.71±1.637)分,对照组为(7.22±1.873)分(t=6.011,P＜0.05);单项依从性中,康复训练(t=5.255,P＜0.05)、规范用药(t=5.516,P＜0.05)、复诊随访(t=3.476,P＜0.05)三项显著高于对照组.结论 延续护理服务能显著提高老年缺血性脑卒中患者出院指导的依从性,研究中建立了规范化的健康教育体系,减少宣教的个体差异,提高护士理论水平,提高了患者满意度.

Influence of continuing nursing care on compliance of discharge guidance of elderly patients with ischemic cerebral apoplexy

Objective To investigate the application effect of continuing nursing care on improving the compliance of discharge guidance of elderly patients with ischemic cerebral apoplexy.Methods 72 elderly patients with ischemic cerebral apoplexy were selected and divided into a control group (n=37) and an intervention group (n=35).The control group were given routine discharge guidance and the intervention group given continuing nursing intervention.A continuing nursing service registration table was formed;the standardized health education system of discharge guidance was established;3-7 days before discharge,the patients were evaluated and health plan and visit record handbook were made.At the end of week 1,4,and 12,discharge interview was taken to record the 5 aspects of secondary prevention.Results At the end of week 12,the total compliance score was (9.71±1.637) in the intervention group and was (7.22±1.873) in the control group(t=6.011,P ＜ 0.05).The compliance score of rehabilitation training,standardized medication,and follow-up visit were higher in the intervention group than in the control group (t=5.255,P ＜ 0.05;t=5.516,P ＜ 0.05;and t=3.476,P ＜ 0.05).Conclusions Continuing nursing care can significantly improve the compliance of discharge guidance of elderly patients with ischemic cerebral apoplexy.In the study,a standardized health education system was established,propaganda and education of individual differences were reduced,and the nurses' theoretical level and patients' satisfaction were improved.
