氟伏沙明联合喹硫平治疗强迫症的对照研究

目的：比较氟伏沙明联合喹硫平与单用氟伏沙明治疗强迫症的疗效。方法：将58例首发强迫症患者随机分为实验组（氟伏沙明联合喹硫平）和对照组（单用氟伏沙明）各29例,两组氟伏沙明剂量均为200～300mg/d;实验组加用喹硫平100～300mg/d;疗程8周。以治疗前后耶鲁布朗强迫量表（Y-BOCS）、汉密尔顿焦虑量表（HAMA）减分率评价疗效。结果：8周末实验组显效率72.4%,对照组为44.8%,差异有统计学意义（χ2=4.549,P〈0.05）。实验组Y-BOCS评分由（22.1±3.1）分减至（10.3±2.6）分,HAMA评分由（12.6±2.1）分减至（7.1±2.7）分,两组治疗后Y-BOCS（t=6.432,P〈0.01）、HAMA（t=3.002,P〈0.01）评分比较,实验组下降更明显。结论：氟伏沙明联合喹硫平比单独使用氟伏沙明治疗强迫症效果更显著。

A control study of Fluvoxamine combined with Quetiapine in the treatment of obsessive compulsive disorder

Objective： To compare the clinical efficacy of Fluvoxamine plus quetiapine and Fluvoxamine alone in the treatment of obsessive compulsive disorder.Methods： 58 patients with first episode obsessive compulsive disorder were randomly devided into experimental group（29 cases） and control group（29 cases）,treated by Fluvoxamine combined with Quetiapine and Fluvoxamine alone respectively.The dose of Fluvoxamine in two groups were 200 ～ 300mg / d,and Quetiapine were 100 ～ 300mg / d,treatment for 8 weeks.The Symptom Checklist of Yale-Brown Obsessive-Compulsive Scale（Y-BOCS） and Hamilton Scale of Anxiety（HAMA） were used to assess the clinical symptoms before and after the treatment.Results： At 8 weekend,the markedly improved rate of experimental group and control group were 72.4% and 44.8% respectively,there was statistical difference（（χ2 = 4.549,P 0.05）.In experimental group,the Y-BOCS scores decreased from（22.1 ± 3.1） points to（10.3 ± 2.6） points,and the HAMA scores decreased from（12.6 ±2.1） points to（7.1 ±2.7） points,and experimental group had a greater improvement than control group after treatment（Y-BOCS： t =6.432,P 0.01;HAMA： t = 3.002,P 0.01）.Conclusion： Fluvoxamine combined with Quetiapine had greater effect than Fluvoxamine alone in treatment of compulsive disorder.
