预置小肠内固定术的适应证和疗效

目的探讨预置小肠内固定术的适应证和治疗作用。方法选择严重腹部外伤、腹腔严重感染、广泛性粘连性肠梗阻,较大或较复杂的腹部手术,特别年老体弱、糖尿病患者,在结束腹部手术后,利用经过我们改制的小肠内固定管做预置小肠内固定术,术后早期经此管肠减压,灌注中药促进肠蠕动,肠功能恢复后行肠内营养。结果本组86例术后平均48～72h肠蠕动恢复,达到了较好的营养支持。无胃瘫、肠麻痹、粘连性肠梗阻等严重并发症发生,恢复顺利。结论选择适应证病例,加做预置小肠内固定术对预防术后严重并发症、促进顺利康复有很好的治疗作用。

Indication and therapeutical effects of pre-placement internal fixation of small intestine:A report of 86 rases

Objective To investigate the indication and therapeutical effect of pre-placement internal fixation of small intestine.Methods Patients with severe abdominal trauma, abdominal cavity infection, extensive adhesive ileus,and subjected to large or complicated operation on abdominal region, were selected,especially in the aged,weak,or diabetic patients. In the end of the abdominal operation, the small intestine internal fixation operation was performed with the small intestine internal fixation tube,which was improved by authors. And in the earlier period after operation, the intestinal decompression, traditional Chinese drug perfusion for enterokinesia promotion, enteral nutrition after the intestinal functional recovery,were also performed by the tube.Results 86 cases were studied in this study,all patients recovered successfully. The enterokinesia recovery time was 48-72 hours, no severe complications as gastroplegia, enteroplegia or adhesive ileus appeared.Conclusion Pre-placement internal fixation of small intestine could prevent severe post-operative complication and promote recovery for the selected cases.
