小骨窗或颅骨钻孔加尿激酶局部注射治疗高血压脑出血疗效探讨

目的　微创手术治疗高血压脑出血的临床疗效。方法　 2 0 0例高血压脑出血患者 ,随机分为内、外科治疗组各 10 0例。外科组采用小骨窗或颅骨钻孔加尿激酶局部注射清除血肿术 ,内科组采用降血压、降颅压及对症处理等常规治疗 ,比较两组的近期 (1个月 )和远期 (6个月 )疗效。结果　外科组治愈率 76 % ,内科组治愈率5 2 % ;外科组病残率 2 0 % ,内科组病残率 5 4 % ,两组相比差异有显著性 (P均 <0 .0 1)。而两组之间的死亡率和再出血率相比差异无显著性。结论　微创手术治疗高血压脑出血较内科保守疗法疗效显著且安全可靠

Clinical effect of small osseons window or sphenotresia plus urokinase therapy on hypertensive cerebral hemorrhage

Objective To study the clinical effect of minimally invasive on hypertensive intracerebral hemorrhage.Methods 200 patients with hypertensive cerebral hemorrhage were divided into medical group and surgical group randomly,each consisting of 100 patients.Small osseous window or sphenotresia plus urokinase therapy was applied to surgical group.Recent(1 month) and late(3 months)follow-up results of the 2 groups were compared with.Results The cure rate of surgical group was 76% and medical group was 52% disablement rate of surgical group(20%)was markedly lower than that of medical group(54%).There were significant differences between the 2 groups(P<0.01). Conclusion Minimally invasive is more effective than medical therapy and has marked clinical result in nerve function recovery of patients with hypertensive cerebral hemorrhage.
