老年冠心病患者的心率变异性分析

目的：探讨老年冠心病患者的心率变异性及其临床价值。方法：应用24h动态心电图对79例老年冠心病患者进行HRV分析，并与68例正常对照者比较。结果：老年冠心病患者HRV的时域和频域指标即SDNN、SDANNI、TI、LF及LF／HF较对照组明显降低（P〈0．01）。而两组RR间期、rMSSD、HF差异元显著性（P〉0．05）。结论：老年冠心病患者存在心脏自主神经失调，老年冠心病患者的HRV降低，表明交感神经活性及迷走神经活性均受损，迷走神经活性受损更为严重，两者协调作用失衡。

Analysis of heart rate variation in old patients with coronary heart disease

Objective To explore the heart rate variation(HRV)and its clinical values in old patients with coronary heart disease(CHD).Method HRV analysis with dynamic electrocardiogram in 24 hours was applied on 79 old patients with CHD,and compared with 68 normal controls.Results The hour area and frequency area index sign of HRV in old patients,namely SDNN,SDANNI,TI,LF and LF/HF were obviously lower than that of the controls(P﹤0.01),but RR,rMSSD,HF did not show the obvious difference(P﹥0.05).Conclusion Old patients with CHD exsit heart independent nerve maladjustment,with lower HRV,and indicate that the activity of sympathetic nerves and pneumogastric nerves all damaged,and the activity of pneumogastric nerves is damaged more seriously,showing the imbalance.
