内支架在老年人左半结肠癌急性梗阻治疗中的应用

目的探讨内支架在老年人左半结肠癌急性梗阻治疗中的临床应用价值。方法93例经确诊为左半结肠癌急性梗阻的患者，48例行术中灌洗一期肿瘤根治术（1组），45例采用放置内支架解除梗阻后再手术（II组），比较两组术后吻合口瘘等并发症发生率、手术时间的差异。结果II组较I组术后吻合口瘘等并发症发生率低，差异有统计学意义（P〈0．05），手术时间比较差异也有统计学意义（P〈0.05）。结论对老年人左半结肠癌急性梗阻应用内支架解除梗阻后再行手术治疗是目前比较可行的方法。

Application of internal stent for left hemicolon obstruction in elder patients

Objective To evaluate the application of internal stent for left hemicolon obstruction in elderly patients. Methods Ninety three patients were diagnosed as obstruction of left hemicolon; among them in one-stage resection were performed in 48 cases （group A）and 45 cases were operated after placing internal stent （group B）. Anastomotic leakage and operation time were compared between two groups. Results The rate of anastomotic leakage in group B was lower than that in group A （P〈0.05）; the operation time was shorter in group B than that in group A （P〈0.05）. Conclusion For elderly patients with obstruction of left hemicolon, the internal stenting before surgery may be the best choice.
