| 摘 要： | 目的探讨乳腺癌改良根治术中保留肋间臂神经的价值。方法72例Ⅰ、Ⅱ期乳腺癌患者随机分为2组,实验组32例保留肋间臂神经,对照组36例行常规乳腺癌改良根治术而切除肋间臂神经,实验组4例因术中未能保留肋间臂神经而退出该组。结果实验组、对照组手术时间分别为(85.3±8.6)min vs.(76.1±7.6)min,术中出血量分别为(71.3±5.6)ml vs.(69.2±6.1)ml,每例检出淋巴结13.2枚vs.14.3枚,术后发生皮瓣坏死、皮下积液、淋巴漏的比例分别为25.0%(8/32)vs.27.8%(10/36),两组比较无统计学差异(P>0.05)。均获随访,平均25.3个月,实验组术后发生上肢感觉障碍6.2%,而对照组高达77.8%,两者比较差异有显著性意义(P<0.01);两组局部复发率为6.2%vs.8.3%,无显著差异(P>0.05)。结论保留肋间臂神经的乳腺癌改良根治术可以明显减少术后患侧上肢感觉障碍发生率且不增加术后局部复发率。
