乳腺浸润性导管癌的病理基础及X线分析

目的:研究浸润性导管癌的X线表现,分析其病理学基础,以提高乳腺癌手术前的诊断水平。方法:23例住院病例,术后病理诊断为浸润性导管癌,结合其病理学基础与生物学特性,回顾分析术前乳腺钼靶X线表现特点。结果:23例浸润性导管癌中,钼钯X线表现:约70%为边缘模糊肿块,70%有毛刺状边缘,而同时出现2~5个间接恶性征象者有20例,占87%。结论:乳腺钼靶X线成像是目前诊断乳腺癌有效的手段。正确识别各种乳腺癌钼靶X线的直接与间接征象是诊断的前提,同时结合临床体查,特别是触诊——对肿块的手感,对提高乳腺癌手术前的正确诊断率非常重要。

Radiological and Pathological Study of 23 Cases of Infiltrating Ductal Carcinoma of Breast

Objective:The aim of this study was to investigate the X-ray features of infiltrating ductal carcinoma and to analysis its pathological basis and to rise the level of the preoperative diagnostic accuracy.Methods:23 continuous cases with infiltrating ductal carcinoma which were confirmed by clinical operation and pathology in hospital were studied. We sorted out their imaging findings into different groups and tried to find out the relationship between these images and behavior of infiltrating ductal carcinoma in patholgy and biology.So we can sum up rules.In some extent,the diagnostic sensitivity and specificity of X-ray examination for breat cancer may be improved. Results:In these 23 cases,about 70% of the margins of tumors is indistinct.In other hand,20 cases of all cases had 2 to 5 malignant signs.the rate was 87%.As a result,combination of more than 2 suspected malignant signs of imaging is more valuable for clinical diagnosis than any single malignant sign. Conclusions:Breast X-ray imaging is the most effective way of preoperative diagnosis for breast cancer at present.But a high diagnostic accuracy is depend on combination of clinical physical examination especially manual feeling of breast palpation and familiarity with direct and indirect radiological signs of breast cancer.
