短期应用瑞舒伐他汀钙对老年扩张型心肌病急性心力衰竭患者血浆基质金属蛋白酶-2和心功能的影响

目的了解短期应用瑞舒伐他汀钙对老年扩张型心肌病急性心力衰竭患者的血浆基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)和心功能的影响。方法将20例老年扩张型心肌病急性心力衰竭患者随机分为研究组和对照组,每组10例,对照组给予常规抗心力衰竭治疗,研究组在对照组基础上加用瑞舒伐他汀钙片治疗,10mg/次,1次/d。分别于用药前、后检测两组患者的血浆MMP-2水平、左室射血分数(LVEF)和左室心肌质量(LVM)并进行比较。结果治疗10d后,两组患者MMP-2、LVEF治疗前后的差值间差异有统计学意义(P<0.01);LVM差值间差异无统计学意义(P>0.05)。结论瑞舒伐他汀钙对老年扩张型心肌病急性心力衰竭患者具有治疗作用;但短期内其不能通过抑制MMP-2生成而产生抗心室重塑作用,可能通过其他机制发挥抗心力衰竭作用,有待于进一步大样本研究。

The Effects of Short Period Use of Rosuvastain on the Level of Plasma MMP-2, LVM and LVEF in Treatment of Old Patients with Acute Heart Failure due to Dilated Cardiomyopathy

Objective To determine plasma matrix metalloproteinase-2 and left ventricular mass and left ventricular ejection fraction by short period use of rosuvastain in treatment of old patients with acute heart failure due to dilated cardiomyopathy.Methods 20 old patients with acute heart failure due to dilated cardiomyopathy were selected and seperated into two groups randomly:experimental group and control group.The patients in experimental group were treated with rosuvastain 10 mg/day besides conventional therapy,the patients in control group were treated only with conventional therapy for 10 days.Before and after rosuvastain pills were given to the patients for 10 days,the level of MMP-2 and left ventricular mass and left ventricular ejection fraction were detected.Results The difference between before theraphy and after theraphy of MMP-2,LVEF has significence(P<0.01),and LVM has no significence(P>0.05)in two groups.Conclusion Statins may have a curative effect on acute heart failure due to dilated cardiomyopathy in old patients,but it did not have an anti-ventricular remolding effect during a short period by inhibit the production of MMP-2.
