应用In-Fast吊带系统治疗女性压力性尿失禁

目的 评价应用In-Fast吊带系统经阴道膀胱颈下吊带耻骨固定术在治疗女性压力性尿失禁中的效果。方法 对2000年12月～2002年1月在我院妇科检查并确诊为压力性尿失禁的女性患者10例，在硬膜外麻醉下采用美国AMS公司的In-Fast吊带系统作经阴道膀胱颈下吊带耻骨固定术。其中中度尿失禁患者8例，重度2例，合并膀胱轻度膨出6例。结果 随访的10例患者于术后近期及术后3个月尿失禁症状完全消失，术后3个月B超检查尿道膀胱连接部移动度平均为9．0mm(术前平均为15．2mm)。漏尿点压力平均为100cmH2O(术前平均为54．2cmH2O)；术后12个月，9例患者尿失禁症状仍完全消失，1例因组织捧异反应予术后6个月取出吊带。结论 In-Fast吊带系统是一种安全、简便、微创的治疗女性压力性尿失禁的手术方法，近期疗效好，但其不良反应还需进行大样本长期研究，并随访其远期疗效。

In-Fast sling for the treatment of female stress urinary incontinence

Objective To evaluate the clinical efficacy of In Fast sling for the treatment of female stress urinary incontinence. Methods From December 2000 to January 2002, ten patients diagnosed as female stress urinary incontinence (six cases combined with slight cystocele, eight with moderate urinary incontinence and two with severe urinary incontinence)underwent transvaginal bladder neck suspension by the In Fast sling (made by AMS company ,USA) fixed to pubis under epidural analgesia.Results Ten cases were followed up with complete disappearance of incontinence symptoms three months after discharge, the mobility of urethrovesical junction measured by B ultrasound was at an average of 9.0 mm(15.2 mm preoperatively), the leak point pressure was 100 cm H 2O (54.2 cm H 2O preoperatively). Twelve months after operation, symptoms of urinary incontinence disappeared completely in nine cases, while the sling was taken out in one case six months after operation because of rejection.Conclusion In fast sling for treatment of female stress urinary incontinence is a safe, simple procedure with mild trauma and good immediate effect. Its long term effect and side effect remain to be further investigated.
