舟骨骨折的诊断、治疗与预后

目的：探讨各型舟骨骨折的预后，以提高舟骨骨折的治疗效果。方法对１１０例舟骨骨折患者进行长期随访，从诊断、治疗上进行综合临床预后分析，从而积累舟骨骨折的诊治经验。结果随访５－１７年，无移位的舟骨骨折７６例，漏诊１５例（１９．７％），发生骨不连１０例（１３％），近端骨缺血坏死１５例术方法治疗１６例，发生骨不连１例；３４例移位性骨折发生骨性关节炎２１例，均为骨不连、畸形愈合、缺血坏死所致。结论移位性舟骨

The diagnosis, treatment and prognosis of scaphoid fractures

Objective To inquire about the prognosis of different scaphoid fractures,so that to improve its treatment. Methods A 5-17 years' follow up was undergone on one hundred and ten cases with scaphoid fractures,from which got a clinical prognosis analysis and rich experience of the treatment of it. Results Among 76 cases with undisplaced scaphoid fractures,miss diagonsis occured in 15 cases( 19.7 %),bone non union in 10 cases(13%),and proximal pole avascular necrosis in 15 cases(19.7%); among 34 cases with displaced scaphoid fractures,18 cases were immobilized by a below elbow plaster,bone non union occured in 6 cases,deformity union in 3 cases,and ischemic necrosis in 6 cases;among 16 cases by operation treatment,bone non union occured in 1 cases; among 34 cases with multi fractures,osteoarthritis developed in 21 cases,all of which was resulted from bone non union,deformity union and ischemic necrosis. Conclusion The incidence rates of bone non union ,avascular necrosis and osteoarthritis are high.For a displaced fractures diffcult to be reduced,an immediate operation is necessary.
