腰椎间盘突出症再手术原因及手术方法的探讨

目的：对腰椎间盘突出症再手术的原因进行分析和探讨，并对再手术临床疗效进行评价。方法：对84例再手术患者进行术前原因分析、影像学辅助检查、体格检查，选择合适的手术方式，术后进行临床疗效评估，并定期随访。结果：腰椎间盘突出复发的主要因素为椎间盘突出、全椎板减压术后腰椎失稳、椎管狭窄、髓核复发等，占73．7％；再手术患者的疗效优良率为81．0％，2次以上再手术发生率为8．3％。结论：腰椎间盘突出症再手术的原因有很多．再手术前需要综合评估再手术的原因和条件，选择适合的手术方式，术后要进行临床疗效评估和定期随访。

Causes analysis and operation methods exploration of reoperation on lumbar intervertebral disc herniation

Obejetive ：To analyze and explore the reasons of reoperation on lumbar disc herniation, and evaluate the operation effect. Methods：To aim directly at 84 reoperation cases, carried out reason analysis before operation, imageology auxiliary examination and physical inspection; chosed suitable modus operandis; evaluated clinical curative effect after operation and took regular follow-up. Results：The main casuses of lumbar disc herniation were intervertebral disk hernia, lumbar instability after whole vertebral plate through decompression procedure, spinal stenosis, nuclues gelationosus recurrence, total taking up 73.7%. The good effect of reoperation patients was 81.0% and above twice operation recurrence rate was 8.3%. Conclusion ：Existing many reasons of prolapse of lumbar intervertebral disc, so we should comprehensively evaluate reasons and conditions of reoperation, then choose suitable operation methods, and take clinical curative effect evaluation and regular follow-up.
