摘    要:目的:分析核磁共振成像(Magnetic Resonance Imaging,MRI)对脑梗死诊断的准确率、病变部位分布及与牛津郡社区卒中计划(Oxfordshire Community Stroke Project,OCSP)分型的符合情况。方法:对52例脑梗死患者进行MRI检查,归纳脑梗死MRI表现,分析其不同发病时间的诊断率,并与OCSP分型进行比较。结果:52例患者均于72 h内明确诊断,诊断准确率为100%,尤以发病6~12 h内确诊率最高(51.92%)。检出病灶94个,其中基底节区病变占34.04%,额叶占22.34%,脑室旁占14.89%,顶叶占11.70%,丘脑占10.64%,其他部位病变占6.38%,以基底节区病变最多。MRI诊断与OCSP分型的符合率分别为完全前循环梗死为88.24%,部分前循环梗死为78.57%,后循环梗死为81.82%,腔隙性梗死为70.00%。结论:MRI对脑梗死患者的诊断率及病灶检出率高,与OCSP分型的符合程度较大。

Abstract:Objective:To analyze diagnosis of Magnetic Resonance Imaging(MRI)in cerebral infarction with the accuracy rate,the distribution of diseased region and the consistency of classification in Oxfordshire Community Stroke Project(OCSP).Method:A total of 52 cases with cerebral infarction received MRI,and performances of MRI in cerebral infarction were concluded,the diagnostic rates of MRI in different periods were analyzed, and the consistencies of classifications in OCSP and results of MRI were compared.Result:Fifty-two patients were diagnosed clearly within 72 hours, and the diagnostic accuracy rate was 100%,especially when the disease was diagnosed in 6 to 12 hours the diagnostic accuracy rate(51.92%)was the highest.A total of 94 lesions were detected,including the lesions of basal ganglia(34.04%),the lesions of frontal lobe(22.34%),the lesions of the ventricle(14.89%),the lesions of parietal lobe(11.70%),the lesions of thalamus(10.64%),the lesions of other parts(6.38%),and lesions in basal ganglia were at most.The coincidence rate of MRI diagnosis and the classification of OCSP before completely circulation infarction was 88.24%, and that in the part of the loop before infarction was 78.57%,in the posterior circulation infarction was 81.82%,in the lacunar infarction was 70.00%respectively.Conclusion:The diagnostic rate of patients with cerebral infarction and the detection rate of lesions by MRI are high,and the classifications of OCSP are in line with results of MRI in the larger degree.
