天津市高中生血清谷丙转氨酶升高与高尿酸血症的关系

摘    要:目的:探讨血清谷丙转氨酶水平与高尿酸血症的相关性。方法:选取健康体检的天津市高三学生(男生963名,女生932名),检测空腹肘静脉血谷丙转氨酶(ALT)、尿酸(SUA)、甘油三酯(TC)、胆固醇(TG)、低密度脂蛋白(LDLc)、高密度脂蛋白(HDL)、空腹血糖(FPG)指标。测量并记录年龄、性别、身高、体重、血压,计算体重指数(BMI)。按照ALT水平将男女分别分为ALT升高组(男性ALT40 U/L,女性ALT35 U/L)和ALT正常组(男性ALT≤40 U/L,女性ALT≤35 U/L)。比较不同性别间ALT升高与高尿酸血症之间的关系。结果:(1)ALT升高人数占总人数的7.65%,男性(11.63%)多于女性(3.54%)(P0.01);单变量分析显示男性ALT升高组与ALT正常组相比,SUA、BMI、SBP、TG、LDL差异有统计学意义(P0.05);女性除上述指标外,还有TC、HDL指标有差异(P0.05),而DBP无差异(P0.05)。男女FPG均无差异(P0.05);随着SUA的升高,ALT升高的人数百分比在男女均升高(P0.01)。(2)线性回归结果显示,在男女中ALT水平与SUA水平呈线性正相关。(方程为男y=0.064x+1.321,R2=0.085,P0.01;女y=0.05x+2.058,R2=0.059,P0.01)。(3)多因素Logistic回归分析显示,高尿酸血症、超重、肥胖(BMI异常)为ALT升高的危险因素,OR值分别为1.956、2.559、9.083(男),2.962、3.456、5.185(女)。而FPG升高和高血压无明显影响(P0.05)。结论:高尿酸血症与肝功能异常有关。

The Relationship between Alanine Aminotransferase and Hyperuricemia among High School Students in Tianjin
Abstract:Objective:To explore the association between alanine aminotransferase(ALT) and hyperuricemia. Method:A total of 1895 high school students aged 17 to 21 who attended the regular health examination were investigated. ALT,serum uric acid(SUA),lipid profiles, fastingplasmaglucose(FPG),etc.They were determined by automatic biochemical analysis device. Weight,height and blood pressure were measure with the same instruments. Subjects were grouped according to ALT level or body mass index(BMI) in both gender. Multiple logistic regression analysis was used to analyze the risk factors of elevated ALT level. Result:The ALT-elevated rate was 7.65%(11.63%)in male and 3.54%in female(P0.05)Conclusion:Hyperuricemia is associated with elevated ALT.
