肝动脉灌注化疗抑制肝癌血管生成的研究

摘    要:目的:通过对肝癌大鼠进行肝动脉区域灌注化疗,研究其对肿瘤血管生成的影响。方法:建立大鼠肝癌模型后,将造模2周后的肝内荷瘤大鼠18只,按照随机数字表法分为对照组、外周静脉组和肝动脉组各6只。大鼠的5-FU给药剂量为20 mg/kg,连续给药4 d。化疗结束后第7天摘取肝脏肿瘤,采用免疫组化法检测VEGF和CD34在肝癌组织中的表达。通过VEGF蛋白染色测定积分光密度(IOD),CD34标记血管内皮细胞检测肿瘤内微血管密度(MVD)。结果:对照组VEGF表达(IOD值)为(46351.48±2926.24),明显高于外周静脉组(43678.84±2852.76)和肝动脉组(35518.86±2657.16),且肝动脉组的肝癌VEGF表达明显低于外周静脉组,差异均有统计学意义(P0.05)。而肝动脉组肝癌组织CD34蛋白表达和MVD最低(36.26±8.43),均明显低于外周静脉组(62.08±10.77)和对照组(69.74±16.60),差异均有统计学意义(P0.05)。而外周静脉组和对照组的CD34蛋白表达和MVD比较差异均无统计学意义(P0.05)。结论:肝动脉灌注化疗能有效抑制肝癌组织VEGF表达,降低MVD,从而抑制肿瘤血管生成。

The Research of Hepatic Arterial Infusion Chemotherapy in Inhibiting Angiogenesis in Hepatocellular Carcinoma

Abstract:Objective:To study the effects on the angiogenesis in hepatocellular carcinoma of rats through hepatic arterial infusion chemotherapy. Method:After the rat hepatocellular carcinoma model was established,18 rats with two-week intrahepatic inoculated tumor were randomly divided into the control group,the peripheral vein group and the hepatic artery group,and six rats in each group.The 5-FU dosage of rat was 20 mg/kg and the rats were administered continuously for 4 days.The liver tumors were removed at the seventh day after the end of chemotherapy,and the expression of VEGF protein and CD34 protein in HCC were detected by Immunohistochemical methods.The value of integrated optical density(IOD)was analyzed by VEGF stained protein,and the tumor microvessel density(MVD)was detected by the marker vascular endothelial cells of CD34.Result:The expression of VEGF(IOD value)in the control group was(46351.48±2926.24),it was significantly higher than that of peripheral vein group(43678.84±2852.76) and hepatic artery group(35518.86±2657.16),and the expression of VEF in hepatic artery group was significantly lower than that of peripheral vein group,the differences were statistically significant(P0.05).Conclusion:Hepatic arterial infusion chemotherapy can effectively inhibit the expression of VEGF and reduce the MVD in HCC tissues,so as to inhibit the angiogenesis of hepatocellular carcinoma.
