超声刀在盆腔淋巴结清扫术中的应用

目的:探讨超声刀在子宫恶性肿瘤手术中的临床应用价值。方法:选取本科2012年9月-2013年3月收治的102例子宫恶性肿瘤患者,行淋巴清扫时根据采用手术器械的不同将所有患者随机分为超声刀组和单极电刀组各51例,超声刀组应用超声刀清扫盆腔淋巴结,单极电刀组应用高频电刀清扫淋巴结,比较两组盆腔淋巴结清扫手术时间、术中出血量、切除淋巴数目、术后引流管放置时间及淋巴囊肿形成上的不同。结果:超声刀组与单极电刀组比较,手术时间明显缩短,术中出血量明显减少,盆腔及腹主动脉旁淋巴结切除数目明显增多,术后引流管留置时间明显缩短,术后淋巴囊肿形成减少,比较差异均有统计学意义(P0.05)。结论:超声刀用于妇科恶性肿瘤盆腔淋巴结清扫手术中,可缩短清扫时间,减少出血量,更完整彻底地切除淋巴结,缩短术后拔管时间、降低淋巴囊肿形成。

The Application of Intraoperative Ultrasound in Pelvic Lymphadenectomy

Objective：To investigate the ultrasonic scalpel in operation of uterine malignant tumors in the clinical application.Method：One hundred and two uterine cancer patients were selected in our department from September 2012 to March 2013.For lymph node dissection according to the different operation equipment of all patients were randomly divided into the ultrasonic scalpel group and the monopolar electrocautery group,51 cases in each group.The ultrasonic scalpel group was treated with ultrasound pelvic lymph node,the monopolar electrocautery group was treated with high-frequency electric knife lymph node dissection.Pelvic lymph node dissection operation time,intraoperative blood loss,number of resected lymph, postoperative drainage tube placement and lymph cyst formation on the different of two groups were compared.Result：The ultrasonic scalpel group compared with monopolar electrocautery group,operation time was significantly shorter,bleeding was significantly reduced,significantly increased the number of pelvic lymph node resection and para aortic lymph nodes,tube indwelling time after drainage operation was significantly shorter,postoperative Lymphocyst formation reduced,the differences were statistically significant（P〈0.05）.Conclusion：Ultrasound knife used for gynecological malignant tumor in pelvic lymph node dissection operation,can shorten the cleaning time,decrease the bleeding amount,more complete and thorough resection of lymph nodes,shorten the postoperative extubation time,reduce lymph cyst formation.
