| 摘 要： | 目的：探讨机械通气对小儿重症手足口病合并神经源性肺水肿（NPE）的治疗作用。方法：回顾性分析16例重症手足口病合并NPE患儿的临床资料。在常规治疗的同时,全部患儿予气管插管使用呼吸机治疗,呼吸方式为间歇正压呼吸方式（IPPV）＋呼气末正压通气（PEEP）。结果：12例患儿肺水肿好转,其中10例患儿成功拔管病情好转,2例进展为多脏器功能衰竭家属放弃治疗;另有4例病情进展迅速于入院后3～17 h死亡。结论：机械通气治疗重症手足口病合并NPE是一种安全有效的治疗方法,能够降低死亡率,PEEP是使用呼吸机治疗NPE的关键。
