子宫内膜癌中PTEN与P27^kipl蛋白表达及相关性研究

目的：研究抑癌基因PTEN和P27蛋白在子宫内膜癌组织中的表达及其与子宫内膜样腺癌（EECA）临床病理因素的关系,探讨二者在EECA发生机制中的作用与相关性。方法：应用免疫组织化学方法检测52例子宫内膜癌（45例EECA）、20例正常增殖期子宫内膜、18例子宫内膜增生症中PTEN与P27的表达情况,并对不同组织学类型和临床病理参数进行分析,同时检验PTEN与P27在EECA中表达的相关性。结果：正常内膜组PTEN的表达明显高于EECA组和增生症组（P〈0.05）,而EECA组与增生症组之间无显著差异（P〉0.05）;P27在正常内膜组的表达显著高于EECA组（P〈0.01）,而与增生症组之间无显著差异（P〉0.05）。Ⅰ、Ⅱ型子宫内膜癌组织中PTEN与P27蛋白的表达有显著性差异（P〈0.05）。在不同临床病理因素中,PTEN蛋白在G1级EECA组织中表达显著高于G2、G3级（P〈0.05）。P27蛋白的表达与年龄及手术病理分期呈明显相关（P〈0.05）。同时,在EECA中P27与PTEN的表达有显著相关性（r=0.418,P〈0.01）。结论：PTEN表达缺失可能是EECA的早期分子事件;P27可能是PTEN介导的细胞周期停滞过程中的下游靶基因,其蛋白表达缺失可能与临床预后相关。

Relativity between expressions of PTEN and P27kipl in endometrial carcinoma

Objective: To investigate the expression of phosphatase and tensin homology deleted on chromosome ten(PTEN) protein in endometrial carcinoma,and its correlation with P27kipl —a presumed downstream target.Methods: Immunohistochemical staining was performed on 52 cases of endometrial carcinoma including 45 endometrioid endometrial carcinoma(EECA),20 cases normal endometrial tissue,and 18 endometrial hyperplasia to detect the expression of PTEN and P27 protein,and the clinic-pathologic relevance was analyzed.Results : The expression rate of PTEN was significantly higher in normal endometrium than that in endometrial hyperplasia and EECA(P<0.05),PTEN expression level was associated with histological grade in EECA;The positive rate of P27 was significantly higher in normal endometrium than that in EECA(P<0.05),P27 expression level was correlated with age and FIGO stage in EECA;PTEN and P27 expression had a positive correlation in EECA.Conclusion: Loss or decrease of PTEN expression may be the early event,and loss or decrease of P27 expression is possibly involed in prognosis of EECA.P27 is considered to be a downstream target of PTEN-dependent cell cycle arrest in endometrial carcinoma.
