盐酸关附甲素和普罗帕酮对室性心律失常疗效的比较

目的:研究盐酸关附甲素注射液治疗室性心律失常的疗效及不良事件.方法:对56例入选的频发室性期前收缩和(或)非持续性室性心动过速的患者进行开放、随机、对照临床试验.试验组42例,给予盐酸关附甲素治疗;对照组14 例,给予盐酸普罗帕酮治疗.结果:入选者性别、年龄、体重及24 h室早数量均无统计学差异.用药6 h疗效(以Holter为准)比较,试验组有效率78.6%,室性早搏抑制率69.5%;对照组有效率28.6%,室性早搏抑制率12.5%.两组有效率比较有统计学差异(P<0.05).试验组用药24 h疗效(以Holter为准), 有效率88.1%,室性早搏抑制率84.7%.试验组用药前后P-R间期、QRS波群、QTc间期无统计学差异(P>0.05).不良反应:试验组10例发生口唇、舌根麻木,1例在负荷量开始时即发生头晕,2例出现发热.对照组6例发生血管反应.均未中断治疗.结论:盐酸关附甲素治疗室性心律失常有显著疗效,且心血管系统的不良反应发生率低.

Comparison of efficacy of Guan-fu base A hydrochloride and propafenone hydrochloride on ventricular arrhythmias

Objective:To study the efficacy and safety of Guan-f ubase A (GFA) on treatment of ventricular arrhythmias. Methods: A randomized, open, controlled clinical trial was conducted in 56 patients withfrequent ventricular premature complex and (or) non-sustained ventricular ta chycardia. Forty-two of them (10 men, 22 women ) receivedGFA, and 14 (5 men,9 women) wereas control treated with propafenone. Results: N o statistical difference was found in sex, age, weight, and ventricular prematur e number of enrolled patients. In trial group, efficacy rate for 6 hours was 78. 6%, and ventricular premature inhibiting rate was 69.5%. In control group, effic acy rate was 28.6%, and ventricular premature inhibiting rate was 12.5%. The eff icacy rate in trial group was higher than that in control group(P<0.05) The efficacy rate of trial group for 24 hours was 88.1%, and ventricular prematu re inhibiting rate was 84.7%. The P-R interval, QRS complex, QTc interval werenot significantly different between before and after treatment (P>0.05) Intrial group, 10 patients had lip and tongue numbness,1 had dizziness, 2 had fev er. In control group, 6 had vessel response. All of the patients received comple te treatment. Conclusion: Guan-fu base A has obvious efficacyin treating ventricular arrhythmia , and incidence of cardiac and systemic sideeffectis low.
