摘    要:目的 探讨MLVA分型对于鉴别布鲁氏菌病复发与重复感染的意义。方法 对初次发病与二次发病的布病患者进行血培养,分离菌株,对分离菌株做布鲁氏菌种属鉴定与MLVA-16分型鉴定,分析两次发病分离到的菌株的分型结果及各位点的差异。结果 收集到4例二次发病的布病患者的血培养物,分离得到8株布鲁氏菌,经MLVA-16分型鉴定为7个基因型,1例两次发病时间间隔较短的患者的2株分离株的MLVA-16分型一致,没有差别,提示为复发。而另外3例两次发病时间间隔较长的患者的6株分离菌株,其MLVA-16分型的基因型不同,分别各有一个位点的差异,提示为重复感染。结论 MLVA分型对于布鲁氏菌病的复发与重复感染具有一定的鉴别意义。

Abstract:To investigate the significance of MLVA genotyping for the discrimination of brucellosis relapse and reinfection. Brucella strains were isolated from the patients with primary and secondary onset of brucellosis. The Brucella Species identification and the MLVA-16 genotyping were carried out for the isolates, the results were then analyzed and the differences between the genotypes and each VNTR were investigated. Eight Brucella strains were isolated from four patients with double onsets of brucellosis. Seven genotypes were identified by MLVA-16 genotyping for the eight isolates. Two strains isolated from one patient with a short time interval of the double onsets was identified as the same MLVA-16 genotype, which indicated that the secondary onset was not reinfection but relapse. While the other six stains isolated from the other patients with a longer time interval of the double onsets were identified as six different MLVA-16 genotypes, and each strain was shown in a difference at one locus. The results indicated that the secondary onset of these three patients were likely reinfection but not relapse. MLVA genotyping of the isolates from the patients with double onsets of brucellosis could provide clue for the discrimination of brucellosis relapse and reinfection.
