外周血T淋巴细胞亚群及血小板活化指标在弥漫性大B细胞淋巴瘤患者中的诊断价值

目的探讨外周血T淋巴细胞亚群及血小板活化指标在弥漫性大B细胞淋巴瘤患者中的诊断价值。方法选取2015年1月—2016年5月我院收治的49例弥漫性大B细胞淋巴瘤患者为观察组,同期49名同龄健康体检者为对照组,检测并比较两组的外周血T淋巴细胞亚群及血小板活化指标水平,并分析不同分期弥漫性大B细胞淋巴瘤患者上述检测结果的差异;采用Logistic回归分析上述外周血T淋巴细胞亚群及血小板活化指标与弥漫性大B细胞淋巴瘤的关系。结果观察组外周血T淋巴细胞亚群指标CD3^+、CD4^+及NK水平均低于对照组,CD8^+及血小板活化指标水平均高于对照组,分期较高的弥漫性大B细胞淋巴瘤患者的CD3^+、CD4^+及NK水平均低于分期较低的患者,CD8^+及血小板活化指标水平均高于分期较低的患者。Logistic回归分析结果显示,CD3^+、CD4^+及NK降低,CD8^+、CD62、GMP-140、CD63及CD61升高均是弥漫性大B细胞淋巴瘤发生的独立危险因素。结论外周血T淋巴细胞亚群及血小板活化指标在弥漫性大B细胞淋巴瘤患者中呈现明显的异常状态,具有较高的诊断价值。

Study on the diagnostic value of peripheral blood T lymphocyte subsets and platelet activation indexes of patients with diffuse large B-cell lymphoma

Objective To investigate and analyze the diagnostic value of peripheral blood T lymphocyte subsets and platelet activation indexes of patients with diffuse large B-cell lymphoma.Methods 49patients with diffuse large B-cell lymphoma in our hospital from January 2015to May 2016were selected as the observation group,49healthy persons of the same age at the same time were selected as the control group,then the peripheral blood T lymphocyte subsets and platelet activation indexes of two groups were detected and compared,and analyzed the differences of detection results in patients with diffuse large B cell lymphoma at different stages,then the relationship between those peripheral blood T lymphocyte subsets,platelet activation indexes and diffuse large B-cell lymphoma were analyzed by the Logistic analysis.Results The peripheral blood T lymphocyte subsets indexes CD3^+,CD4^+ and NK of observation group were all lower than those of control group,the CD8^+ and platelet activation indexes were all higher than those of control group,and the CD3^+,CD4^+ and NK of patients with higher stage of diffuse large B-cell lymphoma were all lower than those of patients with lower stage of diffuse large B-cell lymphoma,the CD8^+and platelet activation indexes were all higher than those of patients with lower stage of diffuse large B-cell lymphoma,and Logistic regression analysis showed that the decrease of CD3^+,CD4^+ and NK,and the increase of CD8^+,CD62,gmp-140,CD63and CD61were independent risk factors for the development of diffuse large b-cell lymphoma.Conclusion The peripheral blood T lymphocyte subsets and platelet activation indexes of patients with diffuse large B-cell lymphoma show abnormal state,and it has higher diagnostic value for these patients.
