NALP3炎性体、骨形态发生蛋白在退变腰椎间盘髓核组织中的表达及意义

摘    要:目的探究退变腰椎间盘髓核组织中NALP3炎性体、骨形态发生蛋白(BMP)的表达及意义。方法在我院选取2017年6-2018年6月接受治疗的80例腰椎间盘突出症患者,患者手术切除标本中取出退变椎间盘髓核组织块,另外选取在我院行前路减压内固定切除的脊柱骨折患者椎间盘的正常椎间盘髓核组织块制作标本待用。对待测标本进行免疫组化染色,使用Western Blot检测NALP3、BMP-2表达。分析NALP3、BMP-2与腰椎间盘突出症患者腰椎间盘退变等级、腰部负荷程度的关系,并分析NALP3、BMP-2之间相关性。结果腰椎间盘组患者中NALP3表达显著高于正常组(P<0.05),BMP-2表达低于正常组(P<0.05);Ⅴ级患者NALP3水平高于Ⅱ级、Ⅲ级、Ⅳ级(P<0.05),BMP-2水平低于Ⅱ级、Ⅲ级、Ⅳ级(P<0.05);重程度患者NALP3水平高于很轻、轻、中程度(P<0.05),BMP-2水平低于很轻、轻、中程(P<0.05);NALP3、BMP-2之间进行Pearson相关性分析,NALP3、BMP-2之间呈负相关(r=-0.269,P=0.016)。结论退变腰椎间盘髓核组织中NALP3升高,BMP-2降低,且与患者退变等级、病情严重程度密切相关,为临床上腰椎间盘突出症的诊治提供理论参考。

Expression and significance of NALP3inflammatory body and bone morphogenetic protein in degenerative lumbar intervertebral disc nucleus pulposus

Abstract:Objective To investigate the expression and significance of nucleotide binding oligomerization domainlike receptor thermoprotein domain 3(NALP3inflammatory body)and bone morphogenetic protein(BMP)in degenerative lumbar disc nucleus pulposus.Methods Eighty patients with lumbar disc herniation who were treated in our hospital from June 2017to June 2018were selected.The degenerative disc nucleus pulposus tissue was removed from the surgical specimens of the patients,and the normal disc nucleus pulposus tissue of the patients with spinal fracture who underwent anterior decompression and internal fixation was selected to prepare specimens for use.Immunohistochemical staining was used to detect the expression of NALP3and BMP-2by Western Blot.To analyze the relationship between NALP3,BMP-2and the degree of lumbar disc degeneration and lumbar load in patients with lumbar disc herniation,and to analyze the correlation between NALP3and BMP-2.Results The expression of NALP3in lumbar intervertebral disc group was significantly higher than that in normal group(P<0.05),BMP-2expression was lower than that in normal group(P<0.05);the level of NALP3in grade V patients was higher than that in gradeⅡ,Ⅲ and Ⅳ patients(P<0.05),and the level of BMP-2was lower than that in gradeⅡ,Ⅲ and Ⅳ patients(P<0.05);the level of NALP3in severe patients was higher than that in very mild,mild and intermediate stages(P<0.05).The BMP-2level was lower than that of very light,light and medium-range(P<0.05).Pearson correlation analysis between NALP3and BMP-2 showed that there was a negative correlation between NALP3and BMP-2(r=-0.269,P=0.016).Conclusion NALP3was abnormally positively correlated with degenerative lumbar disc nucleus pulposus,BMP-2was abnormally negatively correlated with degenerative lumbar disc nucleus pulposus,and was closely related to degenerative grade and severity of the disease,which provided theoretical reference for clinical diagnosis and treatment of lumbar disc herniation.
