应用DALYs评价烟台市恶性肿瘤疾病负担

目的 了解烟台市恶性肿瘤疾病负担现状,为科学制定恶性肿瘤防控政策提供依据.方法 利用2012年烟台市肿瘤登记报告资料和全国疾病监测系统死因监测资料,使用早死所致的寿命损失年(YLLs)、残疾所致寿命年损失(YLDs)和伤残调整寿命年(DALYs)等指标分析恶性肿瘤疾病负担情况.结果 2012年烟台市居民恶性肿瘤DALY率为31.05/千人;男性DALY率(39.12/千人)高于女性(22.96/千人);城区DALY率(25.70/千人)低于县(32.03/千人)和区(31.12/千人);恶性肿瘤DALYs前5位依次为肺癌、肝癌、胃癌、结直肠癌和乳腺癌;恶性肿瘤DALYs随年龄增加而增加.结论 烟台市恶性肿瘤的疾病负担高于全球水平,应针对肺癌、肝癌、胃癌等重点肿瘤和县、区居民,中老年男性等重点人群加强该地区的肿瘤防治工作.

Burden of malignant neoplasms evaluated by DALYs in Yantai city

Objective To evaluate the burden of malignant neoplasms in Yantai City in order to provide scientific basis for policy-making. Methods The registration data from Yantai cancer registry and mortality surveillance in 2012 were reviewed.Years of life lost with premature death(YLLs), years lived with disability(YLDs) and disability adjusted life years(DALYs) were analyzed to evaluate the burden of malignant neoplasms. Results The total burden of malignant neoplasms in Yantai City in 2012 was 31.05 DALYs per thousand persons.The DALYs per thousand males was 39.12 which was higher than that (22.96) per thousand females.There was a lower DALY rate (25.07 per thousand persons) in urban residents than that (32.03 per thousand persons) in county residents and in district residents(31.12 per thousand persons). Lung cancer, liver cancer, stomach cancer,colon-rectal cancer and breast cancer were the top five cancers with the highest health burden.The DALYs of malignant neoplasms increased with age. Conclusion The burden of malignant neoplasms in Yantai population is heavier than that of the international average level. We should pay more attention to the disease control and prevention pointing at the main caners and population.
