全国血站血清丙氨酸氨基转移酶检验室间质量评价

目的：通过血清丙氨酸氨基转移酶（ALT）检验室间质量评价工作，掌握全国血站血清ALT检验状况，使ALT检验逐步规范化，提高血站实验室ALT检验的水平和质量。方法：参照美国AABB／CAP能力验证检验方案，建立我国血站血清ALT检验室间质量评价方案。通过定期向全国参评实验室寄发质控样品，然后对其回报数据进行统计分析，作出实验室检验水平评价，检验方法学评价及试剂盒临床使用评价。结果：从2001年三次质评结果看，获满分的实验室从53．7％提高到64．3％（速率法）、从24．8％提高到48．3％（赖氏法），全年平均得分在80分以上的实验室所占百分比为；ALT（速率法）71．7％，ALT（赖氏法）45．9％。速率法与赖氏法相比较，在检验CV值（精密度）方面，速率法比赖氏法要低约10个百分点；在检验准确性方面，速率法比赖氏法的符合率要高约10个百分点。试剂临床使用质量评价反映出，国产ALT检验试剂质量较差，速率法试剂质量优于赖氏法试剂。结论：开展全国血站血清ALT检验的室间质量评价，有助于提高实验室的检验水平，有助于临床检验试剂盒的选用及检验方法的改进。

The External Quality Assessment of ALT Testing in Blood Banks in China

Objective: Through the external quality assessment for ALT,investigate the national level of examination with ALT, standardize the examination method and improve the testing Quality in blood banks.Methods: The external quality assessment scheme for ALT was established in China according to the AABB/CAP Proficiency Testing scheme for transfusion - transmitted yirus-es. The control samples of ALT were distributed to all laboratories participating the EQA scheme. The data from the laboratories were analyzed statistically. The testing level of laboratories , kit and methods were evaluated. Results: The rate of laboratories obtaining 100 score(PT score) increased from 53.7% to 64. 3% (kinetic measurement) and from 24. 8% to 48. 3% (Reitman - Frankel measurement) in 2001. The rate of laboratories obtaining over 80 score (PT average score) was 71.7% (kinetic measurement) and 45.9% (Reitman - Frankel measurement) in 2001 .The testing CV value of kinetic measurement was lower about 10 percentage point than that of Reitman - Frankel measurement and the testing accuracy of kinetic measurement was higher about 10 percentage point than that of Reitman - Frankel measurement. The quality of domestic kit was lower.The quality of kinetic measurement kit was better than that of Reitman - Frankel measurement kit. Conclusion: The EQA scheme is useful to the improvement of ALT detection level. measurement kit and method in blood banks.
