| 摘 要： | 目的探讨动力髋螺钉（DHS）、股骨近端带锁髓内钉（PFN）治疗老年股骨粗隆间骨折的疗效。方法回顾分析2002年1月～2009年1月225例股骨粗隆间骨折患者临床资料,分别采用DHS（113例）、PFN（112例）进行治疗,比较两者疗效。结果与DHS相比,PFN手术时间短、手术切口长度短、术中出血量少、Barthel生活指数高、住院费用高（P〈0.01）,不稳定型股骨粗隆间骨折髋关节功能Parker评分优良率高（P〈0.05）;但术后并发症、骨折愈合时间以及稳定型骨折Parker评分优良率差异无统计学意义（P〉0.05）。结论治疗股骨粗隆间骨折,PFN疗效优于DHS;不稳定型股骨粗隆间骨折PFN更适合。 |

| Abstract: | Objective To study the significance of internal fixations of intertrochanteric hip fractures in patients by comparing the effects of dynamic hip screw(DHS) and proximal femoral nail(PFN).Methods The therapeutic effects of 225 patients with intertrochanteric hip fractures from Jan.2002 to Jan.2009 were retrospectively reviewed.Results Compared with DHS,PFN had shorter operative incision,operative time,fewer blood loss,higher Barthel Score,higher cost of hospitalization(P<0.01),and higher excellent and good rate in unstable intertrochanteric fracture(P<0.05).There was no statistical significance of postoperative complications,clinical healing time or excellent and good rate in stable intertrochanteric fracture(P>0.05).Conclusion For femoral intertrochanteric fracture,especially unstable intertrochanteric fracture,PFN can be more suitable than DHS. |
