亚低温对心肺复苏脑缺血再灌注损伤后血浆TNF-α的影响

摘    要:目的 探讨亚低温对心肺复苏 (CPR)脑缺血再灌注损伤后患者血浆肿瘤坏死因子 -α(TNF -α)的影响。方法 把 2 2例CPR病人基础生命支持 (BLS)后随机分常规组与亚低温组各 11例 ,用放免法测定各自CPR后 2h、6h、12h、1d、3d、5d血浆TNF -α的变化。结果 ①亚低温组各时点血浆TNF -α含量显著低于常规组 (P <0 0 5 ) ;②常规组血浆TNF -α含量在各时点呈先升后降 ,1d出现峰值 ,亚低温组没有出现峰值。结论 ①亚低温有显著抑制心肺复苏 (CPR)脑缺血再灌注损伤后血浆TNF -α释放作用 ,减轻心肺复苏(CPR)脑缺血再灌注损伤后的急性炎症反应 ,有利于提高脑复苏成功率 ;②TNF -α参与了心肺复苏后脑缺血再灌注损伤的发生和发展过程。

Abstract:Objective To investigate the effect of mild hypothermia on tumor necrosis factor-α(TNF-α) in cardiopulmonary resuscitation(CPR) following cerebral ischemia reperfusion injury. Method 22 patients of CPR following BLS were randomly divided into two groups: routine (11) and mild hypothermia (11)group. The plasma level of TNF-α were measured by radioimmunological assay (RIA) at 2,6,12 hours and 1,3,5 days after BLS. Results ①The contents of TNF-α in mild hypothermia group were significantly lower than that in routine group each time (P<0.01); ②The contents of TNF-α in routine group first up then down, and reached a peak at one day after BLS. Mild hypothermia group have no peak. Conclusions ① Mild hypothermia may significantly lower the contents TNF-α release, decrease acute inflammatory response and raise the chance of success of brain resuscitation. ②The TNF-α may participate in the onset and development of cerebral ischemia reperfusion injury following cardiopulmonary resuscitation
