早期胃癌的病理形态观察及组织发生学探讨

目的 探讨早期胃癌的组织发生学。方法 对１６例早期胃癌及癌周病变进行了病理形态观察。结果 其中２例为多灶性癌，占７５％（１２／１６）。同一病例的各个癌灶互不相连，且各个癌灶的分化程度及组织学类型不同。癌扣伴有没程度的萎缩性胃炎、肠上皮化生、胃腺囊及异型增生。结论 早期胃癌具有多灶性生长的特点。提示临床医生胃镜检查应注意多点取材，以防漏诊；以及正确定手术范围，防止手术残留。『

Study of Pathomorphology and Histogenesis on Early Gastric Carcinoma

Objective To evaluate the histogenesis of early gastric carcinoma(EGC).Methods Pathomorphology of EGC and the peri carcinoma region were observed in 16 cases.Results 12/16 cases(75%) had multiple carcinoma foci which could be different in histological type and differentiation degree.No direct connection were found among them.There were atrophic gastritis,intestinal epithelial metaplasia,gastric glandular cyst and dysplasia in the peri carcinoma region.Conclusion EGC could occur multicentrally.So,it is reasonable that biopsy should not be only focused on one point.And while planning a gastric carcinoma operation,existing multiple carcinoma foci should be kept in mind.
