肝门胆管癌诊治中肿瘤标记物检测的意义

为寻找对肝门胆管癌诊断、疗效判断和随访有价值的肿瘤标记物 ,对 32例肝门部胆管癌和 2 0例胆囊息肉患者行 CA1 9- 9、CA2 4 2 、CA50 和 CEA肿瘤标记物联合检测 ,并对其中 16例行根治性切除术的肝门胆管癌患者进行随访。结果显示 ,CA1 9- 9的敏感性和特异性最好 ,分别为 75 %和 90 % ;4种肿瘤标记物与肝门胆管癌的分期有关 ;肝门胆管癌根治性切除后 ,患者血清 CA1 9- 9、CA2 4 2 和 CA50 水平较术前显著降低。认为血清 CA2 4 2 和 CA50 检测有助于判断术后复发和转移 ;4种肿瘤标记物联合检测对肝门胆管癌术前诊断、疗效评价和随访有价值

Tumor markers of patiens with hilar cholangiocarcinoma

To search for the value of serum tumor markers in diagnosis , evaluating treatment effect and following up of hilar cholangiocarcinoma. Tumor markers CA 19 9 ,CA 242 , CA 50 and CEA were detected in 32 patients with hilar cholangiocarcinoma and 20 patients with gallbladder polyps, and utilized these markers to follow up 16 patients with radical resectable hilar cholangiocarcinoma . Among these tumor markers, CA 19 9 was the best diagnostic index ,its sensitivity and specificity were 75% and 90%, respectively .These tumor markers were associated with ght stage of hilar cholangiocarcinoma . After radical removing tumor, the levels of CA 19 9 ,CA 242 and CA 50 were decreased significantly compared with preoperation .CA 242 and CA 50 are helpful to evaluatint the recurrence and metastasis in postoperation .These results suggest that assay of tumor markers is valuable to preoperative diagnosis of hilar cholangiocarcinoma, valuating treatment effect and following up .
