MSCTA在自发性蛛网膜下腔出血动脉瘤筛查中的价值

目的探讨在急性自发性蛛网膜下腔出血时MSCTA对于颅内动脉瘤筛查的价值。方法回顾性分析20个月来经CT明确诊断的自发性SAH46例患者资料,在急性起病时至一周内行MSCTA,并做数字减影血管造影（DSA）或开颅手术结果比较。结果 46例患者中MSCTA发现阳性38例,一例假阴性,对动脉瘤的检出敏感性为97.44%,特异性为100%;阳性病例显示瘤体形态大小、瘤颈、载瘤动脉及与周围血管解剖关系明确。结论 MSCTA是安全、快捷、无创、可靠的脑血管成像技术,在急性自发性SAH时,是脑动脉瘤首选的筛查方法,同时还能有效的评价瘤体情况,为临床后续处理提供充分的依据。

Value of multidetecter computed tomography angiography in screening intracranial aneurysm of the acute sontaneous subarachnoid hemorrhage

Objective To investigate the multidetecter computed tomography angiography value of screening intracranial aneurysm in the acute spontaneous subarachnoid hemorrhage.Methods The material of 20 months CT diagnosis in 46 patients with spontaneous SAH were reviewed retrospectively.They were detected by MSCTA from acute onset to a week.The results of MSCTA were compared with DSA and neurosurgery.Results 46 cases,MSCTA found positive 38 cases,a causal leave negative,The sensitivity and specificity of detecting aneurysm was 97.44% and 100% ;Positive cases showed occlusion form size,tumor neck,load tumor artery and peripheral vascular anatomy relationship clearly.Conclusion MSCTA is safe,quick,noninvasive,reliable cerebrovascular imaging technology.In acute spontaneous SAH,It is a cerebral aneurysm preferred screening method,at the same time it can also effectively evaluate aneurysmal situation,for clinical follow-up processing to provide adequate basis.
