血浆同型半胱氨酸水平与冠状动脉病变关系的探讨

摘    要:目的研究血浆同型半胱氨酸水平(Hcy)与冠心病的发生及其病变严重程度的关系。方法选取我院行冠状动脉造影检查的患者100例,将其分为冠心病组和对照组。冠心病组为70例经冠状动脉造影证实存在冠心病的患者,对照组为30例冠状动脉造影正常者。测定两组血浆Hcy水平,以Gensini积分系统对冠状动脉病变严重程度进行评估,并结合其他心血管危险因素进行统计学分析。结果冠心病组的血浆Hcy水平明显比对照组高(P<0.01)。Lo-gistic回归显示血浆Hcy是冠心病发生的独立危险因素。多支病变血浆Hcy水平高于单支病变水平(P<0.01)。结论血浆Hcy水平是冠心病的独立危险因素,且升高水平与冠脉病变严重程度相关。

Abstract:Objective To investigate the relationship between blood plasma level of homocysteine (Hcy) and occurrence and severity of coronary heart disease.Methods The total plasma level of homocysteine was determined in seventy patients with coronary heart disease confirmed by coronary angiography and thirty persons with normal coronary angiography for control. The degree of severity of coronary arteriopathy had been assessed by Gensimi scoring system. Other risk factors were analyzed in addition to Hcy and Gensimi's scores.Results The total plasma level of Hcy in patients with coronary arteriopathy was significantly higher than those of normal controls(P<0.01). Logistic regression analysis demonstrated that plasma Hcy was an independent risk factor for the occurrence of coronary heart disease. Plasma level of Hcy had significant correlation with the severity of coronary arteriopathy(P<0.01).Conclusion Plasma level of Hcy is an independent risk factor for the occurrence of coronary heart disease and the increment of its level is closely related to the severity of the disease.
