慢性肾功能衰竭患者血管内皮生长因子测定及意义

目的探讨慢性肾功能衰竭患者血、尿中血管内皮生长因子(VEGF)的作用及其临床意义.方法采用ELISA方法测定31例慢性肾功能衰竭患者(其中失代偿期患者15例,肾功能衰竭期患者16例)和15例正常人的血、尿中的VEGF水平.结果不同阶段慢性肾功能衰竭患者血、尿液中的VEGF水平均明显高于对照组P<0.001,P<0.01.在肾功能衰竭期,血、尿液中的VEGF水平高于失代偿期P<0.01,P<0.001.尿液中的VEGF与肾小球滤过率(GFR)呈负相关,与血肌酐(Scr)和尿素氮(BUN)呈正相关.结论慢性肾功能衰竭患者血、尿液中的VEGF水平增高,可能与肾小球毛细血管损伤的修复增强,保护和维持肾功能有关.肾功能的损害越重,尿液中的VEGF排泌越多,VEGF可作为反映肾功能的一个独特标志.

The assay for vascular endothelial growth factor in patients with chronic renal failure and its significance

Objective To explore the roles of vascular endothelial growth factor(VEGF) in the pathogenesis of chronic renal failure (CRF) and to evaluate the clinical value.Methods The serum and urinary levels of VEGF were examined by ELISA method from 31 patients with CRF (15 cases in the decompensated stage of renal dysfunction,and 16 cases in the end stage of renal dysfunction),as well as 15 health controls were examind.Results In the patients with CRF in different stages,the serum and urinary levels of VEGF were significantly higher than in the healty subjects.The serum and urinary levels of VEGF in the ens stage of renal dysfunction were higher than those in the decompensated stage of renal dysfunction.In additin,negative correlation was observed between the urinary level of VEGF and glomerular filtration rate (GFR).The urinary level of VEGF was positive correlated with serum creatinine(Scr) and blood urea nitrgen(BUN).Conclusions The serum and urinary levels of VEGF in patients with CRF were increased.The may have relationship with process of the repair of injured glomerular capillaries,protecting and maintaining the renal function.The more lesions the renal function are,the more excretion VEGF are.VEGF may be an unique marker of renal function.
