中央型肺癌并阻塞性肺不张的超声诊断

摘    要:目的:探讨超声对中央型肺癌并阻塞性肺不张的诊断价值与影像特征。方法:应用超声盲法诊断中央型肺癌并墼性肺不张患者113例,结果:超声对肺不张的敏感性100%,对不张肺内肿块的敏感性90.3%,对肺不张定位符合率100%,声像特征:(1)单纯型:楔形实质回声区域;肺门肿块及肿块引起的肺不张近肺门处尖端角度圆钝,管状结构的近端截断消失,纵隔向病侧移位,(2)胸水型:相应肋间范围内见到楔形或略有变异的外周为液性暗区包绕内侧与纵隔相延续的实质回声区域;肺门肿块,结论:超声可作为中央型肺癌并阻塞性肺不张时的一种检查手段。

Abstract:Objective:To study the value and image features of diagnosing central type lung cancer complicated by obstructive atelectasis with ultrasonography.Methods:One hundred and thirtecn patients with central lung cancers complicated by obstructive atelectasis were examined by ultrasonography in a prospective study.Results:The sensitivity of ultrasonography to detect atelectasis was 100%,to determine the masses surrouded with atelectasis was 90 3%(102/113).The diagnostic accuracy rate to the lobes of collapse was 100%.The typical appearances of the ultrasonography were:(1)Simple type:The region of atelectasis had a fan like shape,some ducts that distributed from the lung hilus to the circumference radially were seen in 89%(55/62);The masses situated in atelectasis can be seen 92%(57/62)and some of them can cause the atelectasis angle or the duct sections nearby the hilus to become blunt 82%(51/62)or vanish from sight abruptly 71%(44/62);The mediastinum moved to the ill side 100%(62/62).(2)Pleural effusion type:The atelectasis area,of which the shape was fan like or variant,was encircled by the pleural effusion and the inner side of it was continuous with mediastinum;There was masses in the hilus as usual 88%(24/51).Conclusions:The ultrasonography can be served to diagnose central type lung cancer complicated by obstructive atelectasis.The image features above mentioned are useful for that.
