内镜深筋膜下交通支静脉结扎术治疗下肢静脉性溃疡

目的探讨内镜下行小腿深筋膜下静脉交通支离断术治疗下肢静脉曲张的效果。方法回顾性分析14例经静脉造影或多普勒检查确诊为原发性下肢静脉瓣膜功能不全患者，在内镜下行深静脉、浅静脉交通支结扎术的治疗情况。结果术后小腿内侧中下段局部肿胀5例，多在1周内自行消失，切口感染及皮肤烧伤各2例，经治疗后治愈。所有病例浅静脉曲张消失，皮肤色素沉着显著减退，溃疡2～4周愈合。结论内镜下行静脉交通支结扎为彻底治疗静脉交通支功能不全提供了一个良好的治疗手段。

Subfascial Endoscopic Perforator Surgery for the Treatment of Chronic Venous Ulcer

Objective To study the effect of subfascial endoscopic communicating branch vein ablation in treating communicating branch vein insufficiency. Methods 14 cases of primary lower extremity vein valve insufficiency complicated by communicating vein insufficiency diagnosed by venography and color duplex were treated with subfascial endoscopic communicating branch vein ablation , and the related data was analyzed retrospectively. Results Swelling was observed in leg in 5 cases after operation and disappeared within one week spontaneously. Incision infection occurred in 2 cases and skin ambustion happened in 2 cases . Superficial vein varicosis disappeared in all cases, skin pigmentation declined significantly and ulcers healed in 2-4 weeks. Conclusion Subfacscial enddosopic communicating branch vein ablation in an excellent choice in treating communicating branch vein insufficiency.
