肺表面活性物质治疗重症胎粪吸入综合征的疗效

摘    要:目的探讨外源性肺表面活性物质(PS)治疗重症胎粪吸入综合征(MAS)患儿的疗效。方法将40例重症MAS患儿随机分成治疗组和对照组,每组各20例。对照组仅用呼吸机及常规治疗,治疗组在呼吸机及常规治疗的同时应用PS治疗。观察监测二组的肺氧合功能、病程及预后。结果治疗后不同时期,治疗组氧合指数均低于对照组,差异有显著性(Pa〈0.05);动脉/肺泡氧分压比值(a/APO2)高于对照组,差异有显著性(P〈0.05);治疗组机械通气、用氧和住院时间均显著少于对照组(Pa〈0.05)。结论PS治疗能有效改善MAS患儿的肺氧合功能,可缩短应用机械通气及用氧时间及病程。

Abstract:Objective To observe the curative effect of extrinsic pulmonary surfactant(PS) on treating intensive meconium aspiration syndrome(MAS).Methods Forty cases of intensive MAS were divided into treating group and control group randomly,each consisting 20 cases.Cases of control group were treated with respirator and other normal treatment.On the basis of respirator and other normal treatment,cases of treating group were also treated with PS treatment.Pulmonary oxygenation function,course of disease and prognosis of each case were observed and monitored.Results At different stage after treatment,oxygenation index of treating group significantly decreased than those of control group(Pa<0.05);proportion of artery to alveolar partial pressure of oxygen(a/APO2) of treating group significantly increased than that of control group(P<0.05);Time spans of mechanical ventilation,oxygenation and hospitalization of treating group were shorter than those of control group dramatically(Pa<0.05).Conclusions PS treatment can improve pulmonary oxygenation function of pediatric patients of intensive MAS effectively.It can shorten time span of mechanical ventilation and oxygenation and course of disease.
