关节镜和切开手术治疗臀肌挛缩症的疗效对比

目的比较关节镜和切开手术治疗臀肌挛缩症的疗效及关节镜手术技术的探讨。方法2004年10月-2007年7月，对63例123侧臀肌挛缩症行手术治疗，其中34例65侧行关节镜下手术和29例58侧行切开手术。对两组手术时间、抗生素用量、住院时间、切口长度和患者美观满意度、止痛药的运用、术后并发症、康复时间、手术疗效进行对比分析。结果平均随访31．2个月。两组手术疗效和并发症比较无显著性差异妒〉0．05），抗生素用量、住院时间、切口长度和患者美观满意度、术后疼痛和止痛剂的运用、康复时间5个指标有显著性差异P〈0．01）。结论关节镜下软组织松解治疗臀肌挛缩症有切口小而美观，使用抗生素少、术后疼痛轻，功能恢复快，住院时间短等优点，是一种安全、有效、美观的手术方法，值得推广。

Comparison of the therapeutic effects of arthroscopic surgery and surgical incision on gluteal muscle contracture

Objective To compare the therapeutic effects of arthroscopic surgery and surgical incision on gluteal muscle contracture, and to investigate the techniques of arthroscopic surgery. Methods From October 2004 to July 2007, 63 cases （123 laterals） with gluteal muscle contracture underwent operations, including 34 cases （65 laterals） involved in arthroscopic surgery and 29 cases （58 laterals） in incision surgery. Statistical analyses were conducted between the two groups in terms of surgery duration, antibiotic dosage, length of stay, incision size, patients＇ aesthetic satisfaction, analgesic use, postoperative complications, recovery time, and therapeutic effects. Results The mean follow-up was 31.2 months. The two groups showed no significant difference （P〉0.05） in operation effects and postoperative complications. Nonetheless, the following 5 indicators were significantly different （P〈0.01）, which were antibiotic dosage, stay length, incision size and patients＇ aesthetic satisfaction, postoperative pain and analgesic use, and recovery time. Conclusions Arthroscopic surgery for gluteal muscle contracture has advantages such as a better-looking small incision, less use of antibiotics, less postoperative pain, shorter recovery time, shorter length of stay, etc. It is a safe and effective surgical method which can also achieve a better look of the incision for the patient. It is worth promoting.
