电话远程胎儿监护的临床应用

摘    要:目的:评价电话远程胎儿监护系统在临床上应用的价值,探索孕妇家庭自我监护的新途径.方法:选择于妊娠晚期自愿使用远程胎儿监护系统的孕妇118例作为观察组,以远程监护作为自我监护方式;同时选择年龄、孕周及高危因素相似的孕妇120例作为对照组,单纯以胎动计数作为自我监护方式.分别统计两组孕妇的坚持监测率、入院前的心理感受、入院的主动性、胎儿异常发现率及新生儿窒息率和新生儿患病率.结果:观察组孕妇的坚持监测率、入院前的安全感及入院的主动性均明显高于对照组(P<0.05).观察组胎儿异常发现率(10.2%)明显高于对照组(1.7%)(P<0.05),新生儿窒息率(3.4%)明显低于对照组(11.8%)(P<0.05).结论:电话远程胎儿监护系统是一种更准确、更有效、更易为孕妇所接受的家庭自我监护方法,尤其对于高危孕妇,能及时地发现胎儿宫内窘迫,降低新生儿窒息率和病死率,提高围生期保健质量,并具有良好的经济效益,为孕妇家庭自我监护提供了一条新的途径.

Abstract:Objective:To evaluate the clinical value of telephone long-distance electronic fetal heart rate (FHR) home monitoring and to search for a new way of self-monitoring for pregnant women at home. Methods: The study group included 118 pregnant women who volunteered to accept long-distance FHR monitoring as family self-monitoring method. The control group consisted of 120 pregnant women who only used fetal movement counting as family self-monitoring method. The result of non-stress-test (NST) and the rate of fetal distress were compared between the two groups. Result:The rate of abnormal NST in the study group (13.6%) was significantly higher than that in the control group (1.7%,P<0.05). The incidence of fetal distress in the study group (3.4%) was significantly lower than that in the control group (11.8%, P<0.05). Conclusion:The long-distance fetal heart rate monitoring system is a more accurate and efficient family self-monitoring method for pregnant women. It can reveal fetal distress promptly decrease the risk of neonatal asphyxia and fetal death rate. [
