乳腺脂肪坏死的X线诊断

目的 评价乳腺脂肪坏死的X线征象及其病理学基础。方法 回顾性分析14例经病理证实的乳腺脂肪坏死的X线平片，对其中2例活检的病人进行了短期随访。结果 主要X线征象：（1）油脂性囊肿5例，其中2例伴囊壁钙化。（2）不对称性密度增加6例。（3）不规则肿块3例。 14例中8例伴有病灶周围结构紊乱。2例随病灶均无明显变化。结论 乳腺不死的X线表现与其病灶内纤维结缔组织的反应强度有关，在多种多样的X线表现中油脂性囊肿是特异性的X线表现。

Fat necrosis of the breast: findings on mammography

Objective To evaluate the X ray appearance and the basis of histologic findings of fat necrosis in the breast. Methods The X ray films of 14 cases of fat necrosis proved by the histologic findings were reviewed. 2 cases of fine needle aspiration were followed up. Results The spectrum of mammographic features included: (1) Single or multiple lipid cysts ( n =5). (2)Focal areas of increased density ( n =6). (3) Spiculate mass ( n =3). (4) Skin and nipple retraction or thickening ( n =4). (5) Subtle area of architectural distortion ( n =8). There is considerable overlap in the internal characteristics of fat necrosis. In the 2 cases with 3- 6 month follow up, the lesions remained stable. Conclusion The extent of the fibrous connective tissue response can be correlated with the resultant mammographic appearance. Among a wide array of mammographic findings, a lipid or oil cyst had a pathognomonic finding of fat necrosis.
